Prostate arterial embolization: A systematic review

Abstract
Objectives: The purpose of the study was to assess the safety and effectiveness of Prostate Arterial Embolization (PAE) in patients suffering from benign prostatic hyperplasia (BPH) who do not wnat surgical correct of BPH.

Methods: The literature review for PAE was done using 8 national databases including KoreaMed and several international databases including Ovid-MEDLINE, Ovid-EMBASE, and Cochrane Library. Related articles were selected if PAE in patients with BPH. Using strategic search, The subcommittee and two additional reviewers independently performed each step of the assessment including literature search, application of the inclusion criteria, evaluation of the article quality, and data extraction. The quality of the articles were evaluated using SIGN (Scottish Intercollegiate Guidelines Network), and the level of evidence and the grade of recommandation were carefully determined and documented based on the quality.

Results: Out of 237 articles (excluding 94 duplicated articles) selected, 20 (all from international databases) were included in final assessment. The safety of the applied technique could be confirmed, as there was no report on side effects leading to sexual dysfunction, and the severe side effect of bladder ischemia reported by one article was treated immediately and appropriately, and all other reported complications and side effects were of mild level. The applied technique is effective when performed for patients who do not wish to undergo surgical procedures, as there was consistent report on improvement in the intervention group reported by the case studies when pre-surgery and postsurgery (maximum follow-up period) were compared, although the values for maximun urinary flow, post-void residual urine volume, International prostate symptom score were reported to be lower in the intervention
group.

Conclusion: PAE is a safe and effective technique for improving the lower urinary tract symptoms in patients suffering from Benign Prostatic Hyperplasia who do not wish to undergo surgical procedures.
